Inotrope therapy
Inotropes should be considered in a patient with persistent left ventricular failure and hypotension despite an adequate filling pressure (wedge pressure 14-16 mmHg):
- dobutamine infusion:
- inotrope
- 2.5-10 mcg/kg/minute, titrate against response
- dopamine:
- selective renal vasodilator
- initial dose 2mcg/kg/minute
