plozasiran for familial chylomicronaemia syndrome
Plozasiran is an N-acetylgalactosamine (GalNAc)-conjugated small interfering RNA (siRNA) therapeutic that selectively targets apolipoprotein C-III (APOC-3) mRNA to reduce hepatic APOC-3 production and lower plasma triglycerides (1,2,3):
- National Institute for Health and Care Excellence (NICE) guidance TA1204 recommends plozasiran as an option for treating genetically confirmed familial chylomicronemia syndrome (FCS) in adults if response to diet and conventional triglyceride-lowering treatments (such as statins and fibrates) has been inadequate
- compared to the earlier ApoC-III antisense agent volanesorsen, plozasiran demonstrates equivalent clinical effectiveness in fasting triglyceride reduction while offering improved tolerability, a reduced injection burden (quarterly dosing), and no requirement for routine platelet monitoring
NICE Recommendations and Prescribing Criteria
NICE Indications (TA1204)
- recommended as an option for treating genetically confirmed familial chylomicronemia syndrome (FCS) in adults only if:
- response to diet and conventional triglyceride-lowering therapies (e.g., statins, fibrates) is inadequate
- the company provides plozasiran according to the agreed commercial arrangement
- NHS England commissioning: early funding is provided through the Innovative Medicines Fund (from 27 October 2026), transitioning to routine NHS commissioning
Clinical Positioning
- positioning in pathway: offered at the same treatment step as volanesorsen (following inadequate response to a restricted low-fat diet and standard lipid-lowering agents)
- comparative advantage over volanesorsen: similar reduction in fasting triglyceride levels (~50% to 80%), with superior tolerability, extended quarterly dosing, and complete avoidance of routine weekly platelet monitoring
Mechanism of Action and Efficacy
Targeted siRNA Mechanism
- selective APOC-3 mRNA degradation: binds specifically to hepatic APOC-3 mRNA via RNA interference (RNAi), degrading the transcript and inhibiting hepatic synthesis of ApoC-III protein
- dual clearance pathway: lowering ApoC-III releases the inhibition on lipoprotein lipase (LPL) and enhances LPL-independent receptor-mediated hepatic clearance of triglyceride-rich lipoproteins (TRLs), effectively lowering triglycerides even in patients with absent or defective LPL function
Clinical Evidence
- fasting triglyceride reduction: pivotal Phase 3 (PALISADE) and Phase 2 trial data confirm plozasiran reduces fasting triglycerides by 78% to 80% in patients with FCS, lowering levels below the critical 500 mg/dL (5.65 mmol/L) threshold in most participants
- pancreatitis risk reduction: significantly reduces the incidence of acute pancreatitis events in patients with persistent severe chylomicronemia compared to placebo
Dosing, Administration, and Safety
Administration Regimen
- formulation and route: pre-filled syringe/pen administered via subcutaneous injection
- dosing frequency: standard maintenance regimen is administered once every 3 months (quarterly)
- monitoring requirements: unlike volanesorsen, routine weekly platelet monitoring is not required
Safety Profile
- overall safety: generally well tolerated across clinical trials without severe thrombocytopenia or treatment-related discontinuations
- common adverse effects: transient, mild-to-moderate injection-site reactions (e.g., erythema), headache, and slight transient elevations in HbA1c or liver transaminases (ALT/AST)
