- iatrogenic:
- endoscopy
- gastroscopy
- surgery
- non-iatrogenic:
- caustic ingestion
- trauma e.g. knife wound
- foreign body
- carcinoma
- varices
- diverticulitis
- spontaneous perforation - Boerhaave's syndrome
