The first-line thyroid investigation in many hospitals is the highly sensitive TSH assay which should have a limit of detection of <0.1 mU/L.

The addition of an assay for free or total thyroid hormone concentration will reduce diagnostic error but at an increased cost.

Note that reference ranges may vary between laboratories.
