Management of moderate abruptio placentae
The management of moderate placental abruption can be carried out in the following way:
- admit to hospital; consider obstetric Flying Squad
- bed rest
- iv line in situ
- blood: cross-match, FBC, clotting studies
- localize placenta by ultrasound scan
- inspection of cervix with a speculum
There is a danger of premature labour after a bleed of this size; therefore, the patient should not be discharged from hospital. Fetal growth should be monitored.
Labour should be induced at term if it does not occur spontaneously beforehand; labour may be induced earlier if there are signs of fetal distress.
