Testing flexion (C6,7,8) - ask the patient to clench their hands in fists and hold them out in front of them with the backs of their hands facing upwards - tell them not to let you (the examiner) bend his wrists by pushing down on the back of the patient's outstretched fist.

Testing extension (C6,7,8) - ask the patient to resist as you (the examiner) try and pull his wrists up.
