HLA matching is an important determinant of graft survival:
Cadaveric grafts have an average half-life of 8 years. This compares to a half-life of 20 years for HLA-identical graft from a living related donor.
Many deaths are from cardiovascular pathology which often is unrelated to transplantation or to immunosuppression, but acute rejection and early graft loss are becoming increasingly less common
Transplant related death is most commonly due to infection or malignancy.
Survival of the transplant recipient at ten years for cadaveric and live donor transplants is 71% and 89% respectively.
