TUR syndrome
The TUR syndrome is an uncommon complication of transurethral surgery attributed to the absorption of irrigating fluid during the operation and in the immediate postoperative period.
Features include:
- confusion
- hypotension
- bradycardia
- nausea and vomiting
- collapse
Investigations reveal:
- hyponatraemia
- hypovolaemia
- sometimes, hyperammonaemia due to metabolism of absorbed glycine
Management:
- discontinue irrigation
- diuretics
- normal or twice normal saline
- seek senior advice
