Intrinsic reasons:

  • coeliac disease
  • lactose intolerance
  • pancreatic insufficiency, as caused by cystic fibrosis
  • inflammatory bowel disease

Extrinsic reasons:

  • parasitic infestation, for example giardiasis
  • tropical sprue
