The following histological features are found irrespective of the cause of the chronic active hepatitis:

  • intense portal inflammatory infiltrate which spills over into the liver parenchyma
  • piecemeal hepatocyte necrosis
  • bridging necrosis between adjacent triads
  • progressive fibrosis beginning in the portal triads and radiating into the liver parenchyma
  • the end-point is frank cirrhosis
