In the early stages of primary biliary cirrhosis the liver is smooth and enlarged. As the disease progresses nodules appear and finally the liver is grossly cirrhotic. The liver may be bile stained.

Lymph nodes at the porta hepatis are enlarged due to benign reactive hyperplasia. There may be signs of portal hypertension and splenomegaly.
