  • metabolism: - liver function affected by illness, CCF, infections, dehydration, nutrition
  • difficult to identify specific drugs: - renal excretion - GFR falls with age. - particular problems with digoxin, aminoglycosides, lithium, procainamide, tetracyclines.
  • pharmacodynamic factors: - diminished responses to drugs
