Midpalmar space (infection)

Midpalmar space infections are rare and present with:

  • palmar swelling with late fluctuance
  • dorsal swelling due to lymphatic obstruction
  • tenderness and erythema over the site
  • pain on flexor tendon motion

Treatment entails:

  • incision along the distal palmar flexion crease
  • incision into space with culture of released pus
  • irrigation of cavity
  • insertion of drain
  • repeated irrigation if the abscess is extensive
  • delayed closure after 1-2 days

Systemic antibiotics are mandatory.
