The presence of a light reflex with an intact accommodation reflex can be caused by:

  • a midbrain lesion:
    • the Argyll-Robertson pupil due to syphilis

  • a ciliary ganglion lesion:
    • Holmes-Adie pupil

  • Parinaud's syndrome, usually due to:
    • vascular events
    • tumour
