The clinical assessment of binocular diplopia entails:

  • examine eye movements in the six cardinal positions of gaze as in each position, one eye acts predominantly

  • note the direction in which diplopia is maximal

  • cover each eye in turn - the fainter and more peripheral image disappears when the affected eye is covered
