This is an operative technique for limb lengthening based upon the controlled, symmetric distraction of the distal epiphyseal growth plate. It may be used on the femur or the tibia, and in the latter, is accompanied by fibula osteotomy.

A distraction rate of 0.25 mm twice daily appears optimal. Increases of from one third to one half of the original bone length are reported.

The procedure should be performed only in children within 1-2 years of the end of their growth since the operation may jeopardise subsequent epiphyseal growth.
