It is important to note that there may be no history of previous pyelonephritis, indeed urinary symptoms may not be seen.

The clinical features are those of chronic renal damage and insidious renal insufficiency:

  • listlessness, tiredness, vague ill health
  • back pain
  • intermittent fever
  • loss of tubular function results in polyuria and nocturia
