Abdominal pathology:

  • space occupying lesion:
    • cyst
    • haematoma
    • tumour
    • ascites
  • subphrenic infection
  • abdominal surgery

Diaphragmatic pathology:

  • paralysed hemidiaphragm due to phrenic nerve palsy:
    • lung cancer
    • mediastinal cancer
  • diaphragmatic rupture
  • tumour:
    • rhabdomyoma
    • fibroma
  • lax diaphragm

Thoracic pathology:

  • lung fibrosis
  • lung collapse
  • pneumonectomy
  • painful pleuritis
  • pulmonary embolus
  • rib fracture

Conditions which mimic a raised hemidiaphragm include:

  • subpulmonic effusion
  • chest mass
