After palpation of the anterior superior iliac spines, the fingers are advanced superiorly over the surface of the iliac crests. There should be a smooth progression posteriorly as the crest is usually subcutaneous.
If the crests do not lie at the same level, this may be a sign of pelvic obliquity. After palpation of the crests, examination is directed to the iliac tubercles.
