Management
The patient should be resuscitated with fluids.
A laparotomy is required:
- usually a splenectomy is indicated
- in a child as much as possible of the spleen is retained so as to maintain resistance to encapsulated bacteria e.g. pneumococcus and meningococcus
- slices of the spleen may be implanted in the greater omentum in an attempt to retain function
