Epidemiology
- prevalence is approximately 35 cases per 100,000;
- annual incidence of between 5 and 8 cases per 100,000 of population;
- men affected twice as often as women;
- associated with HLA-DR4 and HLA-DQw4; a hereditary form of the disease accounts for 20% of cases and is inherited as an autosomal dominant trait
