The main features are:

  • cardiomegaly and hypertrophy
  • classically there is a disproportionate thickening of the interventricular septum as compared to the free wall of the left ventricle
  • the left ventricular cavity becomes banana-shaped
  • the septum may approach the mitral valve, resulting in left ventricular outflow obstruction, hence the alternative name of hypertrophic obstructive cardiomyopathy or HOCM
