A small and large bowel enema will detect structural abnormalities and tumours. If diarrhoea persists then an aspirate of duodenal fluid should be taken, possibly with small intestinal biopsy to exclude giardiasis, small intestinal protozoa and tropical sprue.

Systemic causes of diarrhoea require special investigations such as:

  • serum VIP determination - Verner-Morrison syndrome; very rare
  • thyroid function tests - thyrotoxicosis
  • fasting blood glucose - diabetes mellitus
  • fasting gastrin level - Zollinger-Ellison syndrome
