The wearing of a corrective prism or an injection of botulinum toxin may be a useful remedy for milder cases of lateral rectus palsy.

Severe cases, or mild cases in which there is no improvement after treatment for 6 months, should be considered for surgery. This involves resection of the paralysed muscle and recession of some or all of the remaining horizontal recti. The insertions of the superior and inferior recti may be transferred to the lateral rectus in total paralysis.

Complete success is possible only with mild cases.
