In oesophageal perforation causing acute mediastinitis, the organisms involved are often those normally found in oral flora, especially anaerobes.
Acute mediastinitis secondary to surgery often involves organisms resistant to the antibiotics used in perioperative prophylaxis. These include Staph. aureus and gram negative bacilli including Pseudomonas aeruginosa.
