Gynaecological conditions which resemble acute appendicitis include:

  • in the non-pregnant female:
    • pelvic inflammatory disease; compared to appendicitis:
      • tends to occur in proliferative phase of cycle
      • pain more generalised
      • longer duration
      • higher fever
      • more pain on displacement of cervix
    • torsion, haemorrhage or rupture of an ovarian tumour
  • torsion of a Fallopian tube
    • Mittelschmerz

  • during early pregnancy:
    • threatened abortion
    • retroverted uterus
    • ectopic pregnancy
    • fibroid degeneration

  • during late pregnancy:
    • labour
    • ruptured uterus
    • abruptio placentae
