Pathogenesis
90% of patients with Felty's syndrome have HLA-DR4.
It appears that the anaemia is a result of the abnormal spleen. Possible explanations include:
- sequestration in the spleen
- humoral factor from the spleen inhibits the bone marrow
- autoimmune destruction of neutrophils with phagocytosis by the spleen
