The investigation of a suspected case of spondylotic cervical myelopathy should include:

  • MRI and CT myelography in neck extension and flexion should show impingement of the thecal sac with obliteration of the CSF space

  • if multiple sclerosis is suspected then MRI of the brain with CSF analysis for oligoclonal bands should be performed

  • motor neurone disease can be investigated with EMG and nerve conduction studies

  • B12 and folate
