Perianal inflammation

Perianal inflammation occurs commonly in Crohn's disease. Conditions that may result from perianal inflammation include:

  • recurrent perianal abscesses
  • Crohns 'piles' - these are pale and oedematous. This is in contrast to the appearance of ordinary haemorrhoids
  • anterolateral anal fissures - usually fissures are posterior anal fissures.

Multiple anal fistulae may develop in Crohn's disease. They may be so numerous as to cause a so-called 'pepper pot perineum' or 'watering can perineum'. This complication is, paradoxically, more associated with small bowel disease than large bowel disease.
