Nodules caused by benign prostatic hypertrophy may be difficult to distinguish from those caused by cancer. The former tend to be more soft and symmetrical, and are found in the inner zone of the prostate. Cancerous nodules are harder, asymmetrical and normally, confined to the outer zone.

Fibrosis following prostatectomy or secondary to chronic prostatitis may produce lesions indistinguishable from cancerous nodules and require biopsy for definition.
