Uterine atony is the commonest cause of postpartum haemorrhage (90%).

Other causes include:

  • retained placenta
  • soft tissue lacerations
  • uterine inversion
  • coagulation disorders eg. von Willebrands disease, idiopathic thrombocytopaenia, liver disease
