The arch of the aorta commences proximally on deep to the right side of the manubriosternal angle - intermediate to the T4 and T5 vertebrae. It passes to the left and posteriorly, first superiorly then inferiorly. The most superior level of the arch is about 2cm below the jugular notch of the manubrium.

The arch joins the thoracic aorta at the level of the T4 vertebra posteriorly. Anteriorly, this is the level of the second costal cartilage on the left.
