Radiographic examination

Authoring team

A neonate presenting with club foot should be examined radiographically at an initial assessment and at each stage of treatment. A 'club foot series' includes both an AP view of the foot and a lateral view of the foot and ankle with the foot passively dorsiflexed.

On the radiograms, club foot demonstrates a marked decrease in the talocalcaneal angle on lateral view, e.g to less than 25 degrees. The AP view shows that a line through the middle of the talus bisects the forefoot much more laterally than normal; a similar line through the calcaneum may pass lateral to the fourth, or even fifth, ray.
