Speech should be assessed for the presence of four features:

  • spontaneity:
    • there may be delay in depressive disorder
    • manic patients may be uninterruptable

  • rate:
    • decreased in depression
    • increased in mania

  • amount:
    • decreased (or absent) in depression
    • increased in mania

  • continuity:
    • speech may lack logical connections (loosening of associations)
    • the patient may flit from one topic to another (flight of ideas)
