The surface landmarks of the posterior superior iliac spines are the surface dimples slightly cephalad to the buttocks. They are readily palpable by feeling posteriorly around the subcutaneous border of the iliac crest.

An imaginary line between the posterior superior iliac spines bisects the sacro-iliac joint at the level of the S2 vertebra.
