Multiview videofluoroscopy

Multiview videofluoroscopy is used in the assessment of velopharyngeal insufficiency as it demonstrates which structures are moving, but more importantly, the degree to which they move. This can be accurately calibrated when combined with simultaneous nasopharyngoscopy. Screening for standard assessment does not expose the patient to a radiation dose of more than 2-4 skull films.

Radiography may be enhanced by the use of contrast media that can be taken as a drink or swallowed as a bolus with semi-solid food.

A number of different view are obtained and this is readily achieved with the C-arm of an image intensier:

  • frontal view: used for assessment of lateral wall motion at the level of the arch of the atlas
  • submento-vertex view: equivalent to the 70 degree oblique endoscopic view in the adult
  • fronto-occipital view: used in children with a relatively horizontal isthmus
  • lateral view: assessment of endoscopic position
