    • high output states
    • increased blood volume - overtransfusion, acute nephritis
  • increased resistance to right ventricular evacuation:
    • pulmonary arterial hypertension
    • pulmonary stenosis
  • inefficient emptying of right ventricle:
    • pulmonary incompetence
    • tricuspid incompetence
