Increased transfer factor occurs in conditions where there is increased exposure of alveolar contents to blood. Thus, the classic example is early in pulmonary haemorrhage, where it is a useful pulmonary function test.

Other situations include

  • left-to-right intracardiac shunts
  • polycythaemia
  • early left heart failure
  • convalescent asthma - in contrast to acute asthma where it is decreased
  • exercise
