About 70% of lumbar disc protrusions occur at the L5/S1 level, thus compressing the first sacral root, and about 25% at the L4/5 level, compressing the fifth lumbar root.

Presentation differs according to whether the protrusion is lateral or central - cauda equina syndrome.
