A carotid endarterectomy should be performed by an experienced surgical team as the long-term advantage of surgery has to be balanced against the perioperative risk.

Morbidity and mortality rates in the best centres are less than 3%, but in less experienced centres they may be in excess of 15%. A successful operation reduces the risk of a stroke to 1 to 2% per annum.
