The cerebral complications of stroke include:

  • cerebral oedema leading to raised intracranial pressure and cerebral herniation. This is an earlier event - 24 hours - in haemorrhagic strokes than ischaemic ones - 4-5 days. Occurs in 41% of cases.

  • haemorrhagic transformation - in 74% of cardioembolic strokes and 30% of ischaemic ones. It may result in acute hydrocephalus or focal neurological deterioration.

  • seizures - usually single and focal. One-third occur within the first 2 weeks, 90% of which occur within the first day. Seizures are found in 11% of cases.

  • depression - seen in half of patients.
