In this rare condition there is an abnormality in albumin which result in increased binding affinity of albumin for T4 (thyroxine).

The increased binding interferes with assays for free and total T4. The patient will be euthyroid but the thyroid function tests will show normal TSH and T3 with apparently elevated T4.
