This is an extreme form of tetralogy of Fallot. The pulmonary valve and right ventricular outflow tract are atretic and right ventricular output is shunted right to left via the VSD into the aorta. Pulmonary blood flow is maintained via the ductus arteriosus and bronchial collaterals.
