This is an avulsion fracture of the spinous process of the seventh cervical vertebrae (vertebra prominens). It is essentially a muscle injury associated with severe muscle contraction - as when shovelling clay ! It is painful but harmless.
Treatment is rest with exercise within the limits of the pain.
