The most common situation is with a blind end oesophagus with a fistula just proximal from the distal oesophagus just proximal to the carina. The trachea is in communication with the lower end of the oesophagus, ie the portion that should have continued from the point of the blind pouch.

A rare variant of tracheosophageal fistula involves communication between an intact oesophagus and the trachea, the so-called H type.
