This is an uncommon condition usually seen in elderly patients. Often, there is an identifiable precipitant.

Presentation is with severe features of hyperthyroidism together with fever, increasing tachycardia and uncommonly, congestive cardiac failure, delirium, coma and death.

The mortality may approach 1 in 10.
