Vertical gaze palsy

A vertical gaze palsy is the loss of upward gaze.

This is discriminated from a third, fourth or sixth cranial nerve palsy because:

  • both eyes are affected
  • pupils are fixed but often unequal
  • generally no diplopia
  • generally the preservation of reflex eye movements, e.g. on flexion and extension of the neck
