Hepatocellular dysfunction leads to enzyme release, necrosis and swelling. This can cause epigastric pain, and, in extreme circumstances, rupture and exsanguination. Occasionally hepatic involvement in pre-eclampsia may lead to jaundice.

The clotting and hepatic problems together cause the HELLP syndrome: H : haemolysis, part of DIC from shredding on fibrin EL : elevated enzymes LP : low platelets
