This uncommon condition results from transplacental passage of stimulating IgG TSH-receptor antibodies to the infant from a mother with a past or current history of Graves' disease.

The maternal concentration of stimulating TSH-receptor antibodies must be high for the neonate to be affected and maternal pre-tibial myxoedema serves a useful marker of the maternal state.
