A hernia through the foramen of Morgagni occurs between the xiphoid and costal margins. They account for 2-4% of all diaphragmatic herniae.

It is usually small and unimportant. Symptoms most often develop in middle life or later. A round or oval, solid or air-filled mass is seen in the middle mediastinum, typically, on the right side. Abdominal contents may be seen in the thorax.

The risk of incarceration necessitates an operation through an upper abdominal incision.
